What's the best way to migrate a table?

Hey, so i got a userbase on my site already and i'm looking for the most effective way to migrate my changes onto the table without clearing the data.

The problem is, if i need to change a column name, add a column etc, i will need to clear the entire database and i don't feel like that's a good in my situation.

 

Any suggestions?

(Note: Using PHP & PostgresSQL)
